Resultados funcionales de la reparación artroscópica del manguito rotador mediante técnica de equivalente transóseo sin nudos

Introducción: las roturas del manguito rotador representan la patología del hombro más común, especialmente en pacientes de edad avanzada. La reparación abierta y artroscópica de esta patología se asocia con excelentes resultados funcionales. Independientemente de los avances recientes en las técnicas de reparación del manguito rotador, la tasa de rerrotura o la falta de cicatrización sigue siendo significativamente alta.
Objetivos: reportar los resultados funcionales de una serie consecutiva de pacientes con roturas del manguito rotador operados con la técnica de equivalente transóseo sin nudos (TOE, las siglas por su nombre en inglés) artroscópica.
Materiales y métodos: se realizó un estudio de cohorte prospectivo entre junio del 2023 y junio del 2024. Se evaluaron todos los pacientes a los que se les reparó el manguito rotador en doble fila utilizando la técnica TOE sin nudos. Se evalúo la movilidad con un goniómetro, el dolor con la escala visual análoga, y los resultados funcionales con los scores de ASES y SANE. Se documentaron las complicaciones postoperatorias.
Resultados: en total se trataron setenta pacientes. El seguimiento medio fue de catorce meses (rango 12–24 meses). Había cuarenta mujeres (57 %) y treinta hombres (43 %). La edad promedio de los pacientes fue de 61.9 ± 4.3 años; todos presentaron una mejoría significativa en todos los scores funcionales evaluados. Además, el 85 % de los pacientes superó la SCB (substantial clinical benefit) para el score de ASES, es decir que tuvieron una mejoría clínica sustancial. En total hubo cuatro complicaciones (6 %) y dos revisiones (3 %).
Conclusión: la reparación artroscópica de las roturas del manguito rotador pequeñas y medianas, tratadas mediante la técnica de doble fila equivalente transóseo sin nudos, se asocia a una mejoría significativa del dolor y los resultados funcionales, con un porcentaje bajo de complicaciones en el corto plazo.
