Arthroscopic Polytetrafluoroethylene Joint Patch for the Treatment of Post-Traumatic Wrist Osteoarthritis

Abstract

Introduction: post-traumatic wrist osteoarthritis can originate from various injuries to the ligaments and bones of this joint. These injuries may lead to altered wrist kinematics, abnormal joint loading, and progressive degeneration of the articular cartilage.

Objective: to report the functional outcomes and pain improvement in a consecutive series of young and active patients with post-traumatic wrist osteoarthritis treated with interpositional arthroplasty using vascular prostheses made of polytetrafluoroethylene (PTFE).

Materials and methods: a retrospective study was conducted on 19 patients with post-traumatic wrist osteoarthritis who underwent an arthroscopic procedure consisting of debridement of damaged cartilage, placement of a PTFE membrane, and application of fibrin sealant between January 2021 and December 2023, as a treatment for post-traumatic wrist osteoarthritis. A visual analog scale (VAS) was used to assess pain during activities, and the Quick DASH questionnaire was administered before and after the surgical procedure.

Results: the mean follow-up was 22.7 months. Statistically significant improvements were observed in postoperative VAS pain scores (from 8.1 to 2.8; p<0.001) and Quick DASH functional scores (from 76.44 to 27.15; p<0.001). The average surgical time was 27.79 minutes. One patient required conversion to total wrist arthrodesis due to poor outcomes.

Conclusion: interpositional arthroplasty using polytetrafluoroethylene (PTFE) vascular prostheses in Young patients with post-traumatic wrist osteoarthritis was associated with significant short-term functional and pain improvement.
