Use of Dual Mobility Cups in Patients with Hip Fracture and Dementia: A 12-Month Follow-Up of 50 Consecutive Cases

Introduction: dementia affects 5% to 8% of people over 60 years of age. Treating patients with dementia and femoral neck fractures is challenging.
Objectives: to describe the 30-day and 1-year rates of dislocation, infection, mortality, and reoperation after total hip arthroplasty with a dual-mobility cup in patients with displaced femoral neck fractures and dementia.
Materials and methods: a single-center, retrospective, observational cohort study was conducted. Fifty patients with hip fractures and dementia who underwent total hip arthroplasty with a dual-mobility cup were identified. The minimum follow-up was 12 months. Preoperative data collected included age at surgery, sex, and American Society of Anesthesiologists (ASA) score. In addition, cup inclination and position were assessed on postoperative anteroposterior radiographs.
Results: no patients experienced dislocation or underwent revision surgery during follow-up. No cases of Deep infection were recorded. Ten patients died within the first 30 days, with a mortality rate of 20% (IC 95%: 10–34%). At one year, mortality rose to 30% (IC 95%: 18–45%), with a total of 15 deaths. The retrospective nature and the inability to assess functional outcomes (inherent to this specific group of patients) were the main limitations of the study.
Conclusion: the use of dual mobility prosthetic systems is an effective and safe option for the management of elderly patients with displaced femoral neck fractures and dementia.
