Impact of Radiographic Bone Quality on Return to Sport After Total Hip Arthroplasty

Abstract

Introduction: total hip arthroplasty (THA) is an effective procedure for improving function and relieving pain, but the relationship between bone quality, physical activity, and return to sport remains poorly understood. The canal–diaphysis ratio (CDR) has been proposed as a simple radiographic marker of bone quality.

Materials and methods: a retrospective study was conducted on 85 patients under 65 years who underwent primary THA between 2019 and 2022. Bone quality was estimated using the CDR, and physical activity was classified according to the static component of the sport. The IPAQ-SF and WOMAC questionnaires were applied.

Results: the mean CDR was 0.45 ± 0.10. A significant interaction between age and type of physical activity was found (p <0.05), with lower CDR values (better bone quality) in patients performing high static load sports. Eightythree percent of patients returned to their previous sport at a median of 60 weeks. Those with higher physical activity levels showed lower WOMAC scores (p for trend = 0.002).

Conclusion: this study suggests a significant relationship between bone quality as measured by the canal-diaphyseal index (CDR), physical activity levels, and return to sport in patients undergoing total hip arthroplasty (THA).
