Resultados clínicos y radiológicos de la reparación parcial de roturas masivas del manguito rotador con aumentación de parche dérmico acelular

Introducción: las roturas masivas del manguito rotador (MR) representan una causa frecuente de dolor y discapacidad funcional. Si bien la reparación artroscópica busca restaurar la biomecánica del hombro, sus resultados pueden verse limitados por la calidad tendinosa y la retracción muscular. En este contexto, la aumentación con parche dérmico acelular (PDA) ha emergido como una estrategia destinada a mejorar la estabilidad estructural y favorecer la cicatrización.
Objetivos: evaluar los resultados clínicos y radiológicos de la reparación parcial del manguito rotador con aumentación de PDA en pacientes con roturas masivas, reparables.
Materiales y métodos: estudio observacional de serie de casos que incluyó 18 pacientes sometidos a reparación parcial del MR con aumentación de PDA. Todos los pacientes completaron una evaluación preoperatoria y seguimiento postoperatorio al mes y a los 2, 3, 6 y 12 meses. Se registraron variables sociodemográficas, rango de movimiento, la escala visual análoga del dolor (EVA) y el puntaje de la American Shoulder and Elbow Surgeons (ASES). El análisis radiológico se realizó mediante resonancia nuclear magnética, valorándose preoperatoriamente el número de tendones comprometidos, la distancia acromiohumeral y las clasificaciones de Hamada, Goutallier y Patte. En el seguimiento se evaluaron los cambios estructurales y la integridad del manguito, definiéndose rerrotura como falla de la reparación.
Resultados: la EVA disminuyó de 7 a 2, mientras que el puntaje ASES aumentó de 55.8 a 85.2 puntos al año de seguimiento. La flexión anterior aumentó significativamente desde el segundo mes y la abducción, desde el tercero. La elevación anterior presentó una disminución inicial seguida de un incremento sostenido a partir del tercer mes. La distancia acromiohumeral aumentó de 6.8 a 8.0 y se evidenció una progresión de Goutallier I a II del supraespinoso. La integridad estructural se mantuvo en la mayoría de los casos, con una tasa de rerrotura del 22 %.
Conclusión: la reparación parcial del manguito rotador con aumentación de parche dérmico acelular mostró mejorías significativas con respecto al dolor, función y movilidad, con preservación estructural favorable al año de seguimiento.
