Osteotomía de la TAT combinada con reconstrucción del ligamento patelofemoral medial en pacientes con luxación recidivante de rótula

Introducción: el objetivo del siguiente estudio fue describir los resultados de una serie consecutiva de pacientes con inestabilidad recurrente de rótula tratados mediante la reconstrucción de ligamento patelofemoral medial (LPFM) combinado con osteotomía del tubérculo anterior de la tibia (TAT).
Materiales y métodos: estudiamos retrospectivamente doce rodillas en once pacientes desde enero de 2017 hasta enero de 2024, con luxación recidivante de rótula, TT-TG de 20 mm o mayor medido con TAC, sin alteraciones en la rotación tibial y/o femoral, y con un seguimiento mínimo de dos años. Evaluamos displasia troclear, altura patelar, lesión condral y rotaciones anatómicas.
Resultados: los promedios fueron, de edad, veintiún años (16-27), tiempo de seguimiento, tres años y medio (2-6) y tiempo de consolidación de la osteotomía de TAT, doce semanas (8-18). Índice de Caton-Deschamps promedio preoperatorio 1.25 y en postoperatorio 0.97. TT-TG promedio preoperatorio 22.9 y en postoperatorio 7.9. Evaluación a los seis meses con scores de Kujala: 93 (75-00), en el score de Lysholm: 92.5 (78-100).
Conclusión: en pacientes con inestabilidad recurrente de rótula y displasia troclear, la osteotomía de TAT combinada con la reconstrucción de ligamento patelofemoral medial (LPFM) resulta en baja incidencia de inestabilidad y se asocia en el mediano plazo a muy buenos resultados funcionales, un alto porcentaje de retorno al deporte y un bajo índice de complicaciones.
