Correction of a Reversed Tibial Slope Using Posterior Closing-Wedge Osteotomy with 3D Planning

Abstract

Anterior tibial slope deformity secondary to malunion of tibial plateau fractures is an uncommon sequela that may result in significant biomechanical alterations. We report the case of a 19-year-old patient presenting with a severe sagittal deformity following previous osteosynthesis, with a tibial slope of 117.8° measured on computed tomography, corresponding to 27.8° of anterior slope relative to the neutral value (90°).

A posterior closing-wedge osteotomy was performed based on three-dimensional preoperative planning, with a planned wedge resection of 28°. Postoperative measurement demonstrated restoration to 90°, achieving complete neutralization of the deformity.

At 6-month follow-up, the patient demonstrated full range of motion (0-135°), return to work, and low-impact physical activity without pain or subjective instability.

This case suggests that, with appropriate patient selection and precise preoperative planning, posterior closure osteotomy with 3D planning can be a valid and reproducible option for the correction of severe reverse tibial slope secondary to malunion.
